Jefferson/Ste 1200 S <br />South Bend, IN 46601 <br />Estimate Number: BMIE-7Z6KCV (2009.4) <br />Customer: <br />Lasalle Building <br />237 N Michigan St <br />South Bend, IN 46601-1202 <br />Attn: Dave Relos <br />Schindler hereby proposes to furnish and install the following with respect to the equipment located at the <br />above building: Freight Elevator <br />Go over operation of each landing door to insure integrity of car operation. <br />Go over every landing door to insure door lock arms are not sticking. (this could allow the elevator to operate <br />without the doors being closed. <br />Go over every landing door and replace door relating gears which are damaged. (these will have to be custom <br />machined as spare parts are no longer available. <br />Add motor bearing oil. <br />We noticed smoke coming out of hoist motor during our inspection. We can not determine if the motor is <br />failing or if the motor bearings are dry. <br />(owner needs to have electric heater directed on hoist machine) <br />Traction safety test, 5-year / Category Five <br />The ASME A17.1 elevator safety code requires a comprehensive elevator safety test, usually on afive-year <br />basis. These tests are conducted to help insure safe and proper operation of the elevator. <br />Schindler proposes to provide all labor, material and supervision required to perform the following scope of <br />work during regular working hours of the elevator trade: <br />- Inspect the governor trip speed and pull-through calibration. <br />- Load the elevator with weights equal to its rated capacity, and test the safety at rated load and rated speed. <br />- Test the buffer at rated load and rated speed. <br />- Test the brake at 125% of rated load. <br />-Inspect and test normal and final stopping devices <br />Page 1 of 4 <br />BMIE-7Z6KCV <br />2009.4 <br />
