Klosinski, Assistant City Engineer SUBJECT: Agenda Request Form — Change Order No. 3 — FINAL Project No. 116-053: Calvert Street Lift Station Replacement DATE: October 16, 2017 The Division of Utilities is requesting the Board of Public Works approval of the Project Completion Affidavit for the quotation project Wastewater Treatment Plant Riverbank Stabilization — Phase 1. EFM Excavating, Inc. has completed all work on time and on budget for this project. There is no Maintenance Bond for the subject work. Plantings (trees, shrubs and grasses) installed along the Riverbank are warranted for a survival rate of 80% per the Plans and Specifications. The Final Project Cost is $89,650.00. The funding sources for the construction of this project is Utilities, Wastewater Contractual Services Other (641-0630-793-63-35).
