SUGGESTED FORMAT <br />(for use with E-500, 2008 Edition) <br />This is EXHIBIT C, consisting of 5 pages, referred to <br />in and part of the Agreement between Owner and Engineer <br />for Professional Services dated Jgly.25, 2017. <br />Payments to Engineer for Services and Reimbursable Expenses <br />Article 2 of the Agreement is supplemented to include the following agreement of the parties: <br />ARTICLE 2 — OWNER'S RESPONSIBILITIES <br />C2.01 Compensation for Basic Services <br />A. Owner shall pay Engineer for Basic Services set forth in Exhibit A at Lump Sum Method of <br />Payment (LSUM) other than services identified as Standard Hourly Rates Method of Payment not <br />to exceed (HNTE). The total shall not exceed $49,800.00 unless an amendment is executed which <br />increases the maximum amount payable. <br />B. Compensation: <br />1. An amount of $49,800.00 based on the following: <br />a. Survey <br />b. Conceptual Intersection Plan <br />c. Road Plan Development <br />d. Geotechnical Services <br />1) Pavement Cores (7 cores) <br />2) Geotechnical Investigation <br />3) Pavement Design <br />e. Utility Coordination <br />f. Bid Period Services <br />g. Construction Phase <br />$ 41,800 (LSUM) <br />5,000 (LSUM) <br />3,000 (LSUM) <br />(HNTE) <br />Total Amount: $ 49,800 <br />Compensation amounts and obligation of the Engineer to perform services for Road Plan <br />Development, Geotechnical Services, Utility Coordination, Bid Period Services and Construction Phase <br />Services described in Part 1---- Basic Services will be addressed in a future supplemental agreement with the <br />Owner.
