EXHIBIT A <br />PROCEDURES FOR REQUESTING THE INSTALLATION OF SCHOOL ZONE SIGNS <br />AND/OR SIGNALS FOR ALL SCHOOLS LOCATED WITHIN THE CITY OF SOUTH BEND <br />In order to objectively and completely consider requests for additional school zone signage <br />and signal installations, the Board of Public Works of the City of South Bend sets forth the <br />following procedures: <br />1. A request, in writing from a school official, is to be submitted to the City Engineer, <br />Engineering Division of the Department of Public Works, The request shall include: <br />* Name of school requesting the sign(s) and/or signal(s) <br />* Name and position of school official making request (to be contact for request) <br />* Contact information (address, phone, e-mail) <br />* Potential locations for signs and/or signals (final determination to be made by <br />City Transportation Engineer) <br />* Any other details deemed pertinent to the sign and/or signal request <br />2. The City Engineer shall evaluate the request and determine the design and location <br />of all signs and/or signals. <br />3. In addition to the standard signing for school zones, additional signs/signals in <br />various combinations may be installed for each school zone: <br />• School Sign (S1-1), with appropriate supplemental plaques as needed for any <br />of its four applications: <br />School Area — warn drivers that they are approaching a school area <br />School Zone — identify the beginning of designated school zone <br />School Advance Crossing — warn drivers that they are approaching a <br />crossing where schoolchildren cross the roadway <br />School Crossing — warn drivers of the location where schoolchildren <br />cross the roadway <br />• Reduced school zone speed limit warning (S4-5 or S4-5a) <br />• School Speed Limit When Flashing (S5-1), with flashing beacon <br />• HAWK Signal — pedestrian actuated crossing signal <br />4. Sign designs shall be in compliance with the current edition of the Indiana Manual on <br />Uniform Traffic Control Devices (IMUTCD) and are not to have any logos or <br />pictographs. Signals, or flashing beacons must meet warrant criteria of the IMUTCD <br />5. Upon a request by the City Engineer, the Bureau of Traffic & Lighting, Streets Division <br />of the Department of Public Works, shall determine the labor and material costs for <br />making and installing the proposed signs. <br />Page I of 2 <br />
