LAFAYETTE — <br />JONES PETRIE RAFINSKI CORPORATION <br />Mr. Gilot stated an application for a Revocable Permit has been received from Jones, Petrie, <br />Rafinski Corporation for the purpose of allowing the installation of an awning at their offices at <br />325 S. Lafayette Blvd. The encroachment shall remain in the right of way until the time as the <br />Board of Public Works of the City of South Bend shall determine that such encroachment is in <br />any way impairing or interfering with the highway or with the free and same flow of traffic. <br />Upon a motion made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the Revocable <br />Permit was approved. <br />TABLE REQUEST FOR REVOCABLE PERMIT FOR ENCROACHMENT 630 N. <br />MICHIGAN ST. AND 420 N. MAIN ST. — NORTH AMERICAN SIGNS INC. <br />Mr. Gilot stated the application has been received for the purpose of allowing the installation of <br />way-finding/directional signs for Memorial Hospital in the grass area between the curb and <br />sidewalk. Upon a motion made by Mr. Gilot, seconded by Ms. Maradik . and carried, the <br />application was tabled due to concerns regarding pedestrian visibility safety. <br />APPROVE TRAFFIC CONTROL DEVICES <br />Upon a motion made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the following <br />traffic control device was approved: <br />NEW INSTALLATION: Residential Handicapped Accessible Parking Space Sign <br />LOCATION: 2309 W. Roger Street <br />REMARKS: All criteria has been met <br />APPROVE WASTEWATER DISCHARGE PERMITS) <br />In a Memorandum to the Board, Ms. Lauren Trapp, Division of Environmental Services, <br />submitted the following Significant Industrial User Permit(s) and recommended approval: <br />New Carlisle and St. Joseph Energy Center — Water Treatment Facility, 54955 Walnut <br />Rd., New Carlisle, IN <br />St. Joseph Energy Center, 54745 Walnut Rd., New Carlisle, IN <br />There being no further discussion, upon a motion made by Ms. Maradik, seconded by Ms. <br />Fritzberg and carried, the permits were approved. <br />APPROVE CLAIMS <br />Ms. Dorau stated that a request to pay the following claims in the amounts indicated has been <br />received by the Board; each claim is fully supported by an invoice; the person receiving the <br />goods or services has approved the claims; and the claims have been filed with the City Fiscal <br />Officer and certified for accuracy <br />Name <br />City of South Bend <br />City of South Bend <br />U.S. Bancorp Assoc. & U.S. Bank National Assoc.; <br />Amount of Claim <br />$5,360,662.16 <br />$1,617,949.18 <br />$45,813.50 <br />Date <br />O5/19/2017 <br />05/23/2017 <br />05/19/2017 <br />
