Appendix "A" is amended as follows: <br />I. CHANGE Task 1 —The CONSULTANT shall prepare an Environmental Assessment (Category <br />Exclusion -Level 2) services for this project. All work shall comply with all National Environmental <br />Policy Act (NEPA), NEPA related regulations and guidelines, and comply with INDOT's <br />Procedural Manual for Preparing Environmental Studies and Categorical Exclusion Manual, <br />2. CHANGE Task 2 — The CONSULTANT shall expand the survey limits to 300 feet north of the <br />centerline of the railroad tracks and prepare a Location Control Route Survey conforming to 865 <br />I.A.C. for the purpose of right-of-way acquisition. <br />3. ADD to Task 5 — The CONSULTANT shall increase the number of soil borings with an additional <br />four (4) borings with an average depth of 10 feet each and incorporate the findings with the <br />previous eight (8) borings with an average depth of 15 feet each and update the completed <br />geotechnical investigation. <br />4. ADD Task 6 — Design Summary/Public Involvement — The CONSULTANT shall set up and hold <br />meetings, when needed, in cooperation with the LPA and INDOT, with local officials, planning <br />agencies or commissions, within the local populace. The CONSULTANT shall prepare <br />presentation materials for hearings or meetings in accordance with "Public Hearing Visual Display <br />Preparation Guide" dated March 1994. The CONSULTANT shall have for project design <br />engineer attend and participate in any such meetings, prepare and distribute minutes, and <br />prepare letters in response to all questions. <br />5. ADD Task 7 — Right-of-way Plan Development— The CONSULTANT shall be responsible for <br />administration, scheduling, and coordinating all activities necessary to certify that the construction <br />is clear for letting including meetings, conferences, communications, with property owners, <br />attorneys, engineers, and INDOT and/or the LPA. <br />6. ADD Task 8 —Construction Phase Services —The CONSULTANT shall be available at <br />reasonable times, during normal working hours, to respond to inquiries concerning the accuracy <br />or intent of the CONSULTANT's plans. On projects involving signing and lighting the <br />CONSULTANT shall review all shop drawings. The CONSULTANT shall also perform utility <br />coordination services during construction. These Utility Coordination Services shall include <br />acting as a liaison between utility companies and the LPA, answering questions, interpreting <br />plans, coordinating activities, and other actions as needed or directed. <br />Appendix "C" is amended as follows: <br />All work by the CONSULTANT under this Contract shall be completed and delivered to the LPA for review <br />and approval within the approximate time periods shown in the following submission schedule: <br />4.
