This proposal is being provided pursuant to Ms. Ann Kolata's <br />electronic mail request on November 1, 2016. The Indiana Department of Environmental <br />Management (IDEM) has issued a No Further Action (NFA) Letter for the site on October 25, <br />2016 (attached). As a condition of the NFA Letter, all monitoring wells must be abandoned in <br />accordance with state statute. <br />PURPOSE <br />The purpose of the work described in this proposal will be to properly seal and abandon nine <br />(25) 2-inch I.D. wells originally drilled for the assessment and monitoring of groundwater. Each <br />of the wells is approximately 30 feet deep (see attached well logs). <br />SCOPE OF WORK <br />WCG will seal and abandon the groundwater monitoring wells consistent with the requirements <br />listed in 312 IAC 1340-2. These wells were installed during the course of the remedial work <br />completed at the referenced site. The process will include sealing of the wells using bentonite <br />chips manufactured specifically for water well sealing purposes, and the removal of the well <br />casing to a depth of at least 2 feet below existing grade.
