Klosinski, Assistant City Engineer <br />SUBJECT: Agenda Request Form — Change Order No. 1 & Project Compl. Affidavit <br />Project No. 116-051 WWTP Belt Filter Press #3 Rehabilitation <br />DATE: February 3, 2017 <br />The Division of Utilities is submitting Change Order No. 1 and the Project Completion <br />Affidavit for Project No. 116-051 WWTP Belt Filter Press #3 Rehabilitation to the Board <br />of Public Works for review and approval at the February 14, 2017 Board of Public <br />Works Meeting. <br />Alfa Laval has completed all work on Belt Filter Press #3. The $5,000 allowance for <br />unforeseen issues and miscellaneous repairs was not needed during the course of the <br />work. Therefore, Change Order No. 1 addresses this as a credit back to the City of <br />South Bend since the funds were not used during construction. The result is a -2.60% <br />decrease to the final contract price for the work. <br />The funding source for this contract is Utilities, Wastewater Repair and Maintenance <br />Other Equipment (641-0630-793-36-05). <br />If there are any questions regarding the subject project, please contact Al Greek (Ext. <br />4210) or Jacob M.
