St. Joseph County Housing Consortium <br />• Spiritual Development <br />• Case Management, mentoring and goal-setting <br />• Mental and Physical Healthcare <br />• Adult Education and GED completion <br />• Financial Planning <br />• Hope4Kids Early Childhood Development Program and Parenting Programs <br />• Recovery of Hope Substance Abuse Recovery Program <br />• Employment Readiness skill development <br />• Life Skills classes <br />• Connection to community services <br />Madison Center <br />Madison Center, a comprehensive community mental health center, is a private nonprofit <br />agency located in South Bend. Madison Center serves St. Joseph, Elkhart, Marshall, La <br />Porte and Porter counties. Madison Center provides a full array of services to children, <br />adolescents, families, adults and geriatrics. There are two programs that target the <br />homeless population and both of these programs are housed at the Center for the <br />Homeless. One of the programs is the Community Support Program (CSP). This program <br />provides case management services to the seriously mentally ill population that are <br />homeless or who have a history of homelessness. This program is able to complete <br />outreach and assessment of the clients who are homeless and mentally ill. New Passages is <br />the other program on-site at the Center for the Homeless. This is an intensive outpatient <br />alcohol/drug treatment program that serves dually diagnosed individuals who may be <br />homeless. <br />Along with these programs, Madison Center is also the recipient of Supportive Housing and <br />Shelter-Plus Care grants to assist homeless clients in obtaining housing. Below is a listing <br />of those grants along with the number of spots on the grant. <br />Supportive Housing Program 15 <br />Shelter Plus Care-CSP Scattered 10 <br />Shelter Plus Care-Scattered site 13 <br />Shelter Plus Care-Gateway/Families 25 <br />Supportive Housing-Turnock House 8 <br />Madison Center owns one apartment building (Gateway) that is funded entirely by one of <br />the Shelter Plus Care grants. This apartment and funding through Shelter Plus Care <br />provides permanent housing to the formerly homeless individuals. While living here, clients <br />receive case management services through another Madison Center case management <br />program, Semi-Independent Living Program. <br />Madison Center has one group home that receives Supportive Housing Program assistance. <br />This group home, Turnock House, has space for eight individuals to live and receive 24-hour <br />support through the staff on-site. <br />Youth Service Bureau <br />The Youth Service Bureau of St. Joseph County, Inc. offers a number of services that <br />positively affect the lives of runaway, homeless, and in crisis youth ages 12 through 24 <br />years old. These services include an active Street Outreach Program; Safe Station, which is <br />an emergency shelter for runaway and homeless youth ages 12 through 18 years old; a self <br />sufficiency program for young moms and their children; and an apartment based <br />transitional living program for runaway, homeless, or in crisis young mothers and their <br />children. <br />2010-2014 Housing and Community Development Plan 51 <br />
