It was also recently brought to our attention that the original patch panels do not have the inputs <br />and spares that the City wants. This pricing is to furnish and installed three LC fiber patch panels. <br />The following changes to the Contract are proposed: <br />SCOPE OF WORK: (attach and list supporting information as required) <br />1. Item: Wunderlich-Malec — See attached breakdown - $7,500 <br />2. Item: TCI — PM 2hrs @ $125/hr, PE 2hr @ $75/hr, PC 1 hr @ $50/hr <br />JUSTIFICATION: <br />1. Item: Specification Discrepancy/Owner Request <br />CHANGES IN CONTRACT PRICE AND CONTRACT TIMES: <br />We propose that the Contract Price and Contract Times be changed as follows: <br />For Contract Price, when requested by Engineer, attach detailed cost breakdowns for Contractor and <br />Subcontractors, Supplier quotations, and other information required. <br />For the Contract Times, state increase, decrease, or no change to Contract Times for Substantial Completion, <br />readiness for final payment, and Milestones, if any.
