CLAIMS REVIEW MEETING NOVEMBER 15,201 359 <br />A Claims Review Meeting of the Board of Public Works was convened at 9:02 a.m. on Tuesday, <br />November 15, 2016, by Board Member Elizabeth Maradik with Board Members Therese Dorau <br />and James Mueller present. Board President Gary Gilot and Board Member Suzanna Fritzberg <br />were absent. Also present was Attorney Michael Schmidt. The meeting was held in the 13' Floor <br />Boardroom, County -City Building, 227 W. Jefferson Blvd., South Bend, Indiana. <br />APPROVE CLAIMS <br />Ms. Dorau stated that a request to pay claims has been received by the Board; each claim is fully <br />supported by an invoice; the person receiving the goods or services has approved the claims; and <br />the claims have been filed with the City Fiscal Officer and certified for accuracy. Bank National Association; 2015 EDIT/Parks <br />Bond; Requisition No. 29 <br />$10,002.74 <br />I1/11/2016 <br />PNC Equipment Finance, LLC & U.S. National Bank; <br />CNG Conversions; Requisition No. 11 <br />$14,370 <br />11/11/2016 <br />PNC Equipment Finance, LLC & U.S. National Bank; I $113,732 <br />T&L Aerial Tower; R uisition No. 10 <br />11/11/2016 <br />Therefore, Ms. Dorau made a motion that after review of the expenditures, the claims be approved <br />as submitted. Mr. Mueller seconded the motion, which carried. <br />APPROVE DEED OF DEDICATION — SOUTH BEND HERITAGE FOUNDATION, INC. <br />Mr. Richard Estes, Engineering, presented a Deed of Dedication for the Board's approval. In a <br />Memo to the Board, he explained this is to correct issues from a previous dedication from the <br />South Bend Heritage Foundation to the City at the southeast comer of Indiana Avenue and Kemble <br />Street that never got recorded. Mr. Estes noted this is to update right-of-way boundaries along <br />these streets. Attorney Schmidt stated he has reviewed the dedication and recommends the Board <br />aceeptthe property. Therefore, upon a motion by Ms. Dorau, seconded by Mr. Mueller and carried, <br />the deed was approved. <br />ADJOLRI MENf <br />There being no further business to come before the Board, upon a motion by Ms. Maradik, <br />seconded by Ms. Dorau and carried, the meeting adjourned at 9:05 a.m. <br />BOARD OF PUBLIC, WORKS <br />Gary A.
