4R, <br />ONES PETRIE RAFINSKI <br />October 27, 2016 <br />City of South Bend <br />Division of Utilities <br />3113 Riverside Drive <br />South Bend, IN 46628 <br />Attn: Ms. Andrea Alexander, Manager of Environmental Compliance <br />RE: REVISED SURVEYING SERVICES FOR LAGOON NO.4 AT THE ORGANIC RESOURCES FACILITY <br />ON OLD CLEVELAND ROAD, SOUTH BEND, INDIANA <br />Dear Ms. Alexander: <br />Thank you for contacting JPR regarding this project. As you are aware JPR performed topographic surveying <br />services for this site back in 2007/2008. It is our understanding that you are seeking a proposal for surveying <br />services for the Lagoon No. 4 site located that the Organic Resources facility on Old Cleveland Road. Per your <br />request via email on Friday, October 21, 2016 we have discussed the survey limits with Tony Maxson with <br />Weaver Consulting. JPR is to collect topographic information along the east side toe of slope of Lagoon No. 4 <br />including an additional 100 feet to the east of the Lagoon as well the south side toe of slope to the fence line <br />around the airport property. Mr. Maxson also requested that we locate any vent pipes and wells within these <br />survey limits. Furthermore, it is understood that you are requesting that the east boundary line of the facility be <br />marked for the installation of an additional well. JPR can mark this line within the limits of concern per the aerial <br />image provided in your email on October 24, 2016. <br />JPR is pleased to provide you this proposal for preparing a Topographic Survey for the limits outlined above. <br />The following tasks will be performed by JPR in order to complete the Topographic Survey drawing: <br />• Establish horizontal and vertical control within the project limits <br />• Collect topographic information along the east and south side of the existing Lagoon No. 4 <br />• Set lath marking the east boundary line within the area of concern as outlined in you October 24th email <br />• Prepare and final drawing of the existing site conditions and provide a paper copy, pdf and CAD files of <br />the surveyed area <br />Limitations: <br />• This proposal does not include performing utility locates. Should you desire to have the utilities located <br />JPR can include this service at an additional cost. Utility locates are requested utilizing the 811 Call <br />Before You Dig phone number. Once a ticket has been requested the utility locate company has 48 <br />hours from the time the ticket is issued to mark the project limits. Only utilities that are members of the <br />811 service are marked. Any non-members would need to be notified to mark their respective utility. <br />The fee for preparing the Topographic Survey is a not to exceed fee of: $4,510.00. <br />Work can begin upon receipt of a signed copy of this proposal, and will be completed within 15 calendar days or <br />sooner, barring unforeseen delays, such as adverse weather conditions, of which you would be notified.
