C. <br />ii. <br />Registry Management <br />The Registry shall be maintained at the office of the Sewer Insurance Administrator, with a <br />copy held by the Clerk for the City's Board of Public Works. <br />The Registry shall consist of three separate pre -qualification categories. The categories and <br />pre -qualification requirements are as follows: <br />Definition <br />Second Opinion Those contractors who are <br />to evaluate and perform <br />immediate repair work to a <br />Private Sewer <br />Excavation <br />Those contractors who are able <br />to perform repair work to a <br />Private Sewer through open cut <br />excavation to a maximum depth <br />of nine feet. <br />NOTE: benching a trench excavation <br />shall only be authorized as an <br />approved method oftrench excavation <br />by the Sewerinsurance Administrator <br />on a case by case basis and will not be <br />assumed as an authorized practice or <br />technique for pre -qualification. <br />Pre -qualification <br />Equipment: <br />• CCTV system (optional) <br />• Cable machine; <br />o Industrial gear driven <br />device or equivalent; <br />0 3/ HP motor <br />o '/" cable minimum size; <br />ability to clean a minimum <br />of 180' <br />o Four blade cutter (2", 4") <br />o Sharktooth cutter (2", 4" <br />& 6") <br />o Expanding cutter(s) (up to <br />6" expansion) <br />o Blow bag (optional) <br />o Retrievers (big & small) <br />Experience: <br />Demonstrate experience through <br />reference covering use of <br />required equipment on sewer <br />line repair with a cable <br />machine for lines un to 6" <br />• Trench box system to <br />provide trench safety per <br />OSHA at a trench width of <br />four feet (outside width) <br />• Excavation equipment <br />sufficient for a trench width <br />of no more than three feet <br />wide and eight feet deep. <br />Experience: <br />Demonstrate experience through. <br />reference regarding sewer line <br />repair trough open cut <br />excavation and appropriate post - <br />construction property <br />restoration. <br />Safety: <br />OSHA confined space training <br />
