REGULAR MEETING Douglas L. Carpenter & Associates, Inc. 922 East Jefferson South Bend, Indiana City -Owned Property: City's Offering Price: Bid: MAY 24, 1982 1125 & 1129 S. Lafayette $1,400.00 $1, 400 .-00 Upon a motion made by Mr. Kernan, seconded by Mr. Hill and carried, the bid of Douglas L. Carpenter & Associates in.the amount of $1,400.00 was accepted and the matter referred to the Legal Department for drafting of the appropriate purchase agreement. Douglas L. Carpenter & City -Owned Property: 311 Stull Associates, Inc. 922 East Jefferson City's Offering Price: $190.00 South Bend, Indiana Bid: $190.00 Upon a motion made by Mr. Kernan, seconded by Mr. Hill and carried, the bid of Douglas L. Carpenter & Associates in the amount of $190.00 was accepted and the matter referred to the Legal Department for drafting of the appropriate purchase agreement. It was noted that no bids for the purchase of the city -owned property located at 1227 W. Colfax had been received. AWARD BID AND APPROVE CONTRACT - CHAPIN AND WESTERN RADIUS IMPROVEMENT PROJECT (LA FREE EXCAVATING COMPANY) Mr. Neil Shanahan, Manager of the Bureau of Public Construction, recommended the Board award the low bid of LaFree Excavating,'Inc. in the amount of $15,772.35 for the Chapin and Western Radius Improvement Project (82-3). Bids for said project were received by the Board at its meeting held on May 17, 1982. Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, the recommendation to award and the contract for performance of the work by LaFree Excavating were approved, and the required Performance Bond, Labor and Materials Payment Bond and Certificate of Insurance were filed. APPROVE CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - "OLD" CLEVELAND ROAD SANITARY SEWER PROJECT (DYE PLUMBING) Mr. Leszczynski advised that Dye Plumbing and Heating Company had submitted Change Order No. 2 (final) in the amount of an increase of $7,930.77, thereby bringing the final contract cost to $412,648.37,for the "Old'' Cleveland Road Sanitary Sewer Project.. Also submitted was the Project Completion Affidavit for the project indicating a maintenance agreement running to May 24, 1985. Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, the above Change Order No. 2 and the Project Completion Affidavit were approved, subject to the filing of the appropriate three-year Maintenance Bond. APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - SOUTHERNVIEW STEEL CULVERT PROJECT (KOSANOVICH & SONS EXCAVATING) Change Order No. 1 (final) in the amount of an increase of $1,042.28 was submitted by Kosanovich & Sons Excavating on the Southernview Steel Culvert Project, thereby bringing the final contract cost to $7,308.75. Also submitted was the Project Completion Affidavit for the project indicating a maintenance agreement running to May 24, 1985. Upon a motion rude by Mr. Leszczynski, seconded by Mr. Hill and carried, the above Change Order No. 1 and the Project Completion Affidavit were approved, subject to the filing of the appropriate three-year Maintenance Bond.
