D114ve <br />MAY 17, 1982 <br />0 <br />7 <br />REGULAR MEETING <br />The regular meeting of the Board of Public Works was convened <br />at 9:30 a.m., on Monday, May 17, 1982, by President John E. <br />Leszczynski, with Mr. Leszczynski, Mr. Richard L. Hill and <br />Mr. Joseph E. Kernan present. Also present was Assistant <br />City Attorney Eugenia Schwartz. <br />MINUTES OF PREVIOUS MEETING APPROVED <br />Upon a motion made by Mr. Kernan, seconded by Mr. Hill and <br />carried, the minutes of the May 10, 1982, regular meeting of <br />the Board were approved as submitted. <br />OPENING OF BIDS -RADIUS IMPROVEMENTS (NORTHWEST <br />CORNER CHAPIN AND WESTERN) <br />This was the date set for receiving and opening sealed bids for <br />the construction of radius improvements at the northwest corner <br />of Chapin and Western. The Clerk tendered proofs of publication <br />of notice in the South Bend Tribune and the Tri-County News which <br />were found to be sufficient. The following bids were opened and <br />publicly read: <br />Rieth-Riley Construction Bid was signed by Richard D. Ruhlman, <br />P. 0. Box 1775 non -collusion affidavit was in order, <br />South Bend, Ind. 46634 and a 5% bid bond was submitted. <br />$19,577.75 <br />Arco Engineering Construction Bid was signed by William Zirkle, <br />705 S. Beiger Street non -collusion affidavit was in order, <br />Mishawaka, Ind. 46544 and a 5% bid bond was submitted. <br />$24,804.00 <br />Ziolkowski Construction, Inc. Bid was signed by Ben Ziolkowski, <br />1005 S. Lafayette non -collusion affidavit was in order, <br />South Bend, Ind. and a 5% bid bond was submitted. <br />$17,191.75 <br />LaFree Excavating, Inc. <br />11252 West Third St. <br />Osceola, Ind. 46561 <br />$15,772.35 <br />Murdock & Sons Construction <br />1320 S. Franklin St. <br />South Bend, Ind. 46618 <br />$18,846.05 <br />Bid was signed by Richard LaFree, <br />non -collusion affidavit was in order, <br />and a 5% bid bond was submitted. <br />Bid was signed by Calvin Murdock, <br />non -collusion affidavit was in order, <br />and a 5% bid bond was submitted. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Kernan and <br />carried, the above bids were referred to the Engineering Department <br />for review and recommendation. <br />APPROVE CHANGE ORDERS AND CERTIFICATE OF SUBSTANTIAL <br />COMPLETION - FIRST BANK PROJECT <br />Mr. Leszczynski advised that numerous change orders, which had <br />been reviewed by the construction manager, architect and Patrick <br />McMahon for the First Bank Center Project, were being submitted <br />to the Board for approval. Upon a motion made by Mr. Leszczynski, <br />seconded by Mr. Hill and carried, the following change orders and <br />certificate of substantial completion were approved <br />Hunzikers, Inc. - Contract No. 125A, Exterior Landscaping <br />Addition of $18,490.00 for installation of slag stone in <br />various locations, additional plantings, timber retaining <br />wall and additional sod and netting (Change Order No. 1). <br />Revised contract amount: $107,226.00 <br />
