REGULAR MEETING <br />JANUARY 10, 1983 <br />APPROVE PROJECT CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVIT - MONROE/SAMPLE REDEVELOPMENT, PHASE II (RIETH-RILEY) <br />Mr. Leszczynski advised that Rieth-Riley Construction Company <br />had submitted Change Order No. 2 (Final) indicating that the <br />contract amount will be decreased by $231.95 for a new contract <br />sum including this change order in the amount of $262,113.34. <br />Additionally submitted was the Project Completion Affidavit <br />indicating this new final cost. Upon a motion made by Mr. <br />Leszczynski, seconded by Mr. Hill and carried, Change Order <br />No. 2 and the Project.Completion Affidavit were approved. <br />APPROVE REQUEST TO ADVERTISE FOR BIDS - UNIFORM RENTAL <br />Mr. Kernan requested permission to advertise for the receipt of <br />sealed bids for the rental of uniforms for various city depart- <br />ments. Mr. Kernan indicated that the present two (2) year <br />contract expires at the end of the month and the contract <br />provides for the rental of uniforms for hourly city employees. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill <br />and carried, the request was approved and a bid date of <br />January 31, 1983 established. <br />UEST FOR ROAD RUN - ST. ADALBERT/CORVILLA RUN REFERRED <br />A letter was submitted to the Board from Mr. William Michalski, <br />requesting approval of the 5th Annual St. Adalbert/Corvilla <br />Fun Run to be held on Sunday, July 17, 1983 at 6:00 p.m. <br />through the streets of the west side. Upon a motion made by <br />Mr. Leszczynski, seconded by Mr. Kernan and carried, the <br />above request was referred to the Bureau of Traffic and <br />Lighting and the Police Department Traffic Division for <br />review and recommendation. <br />APPROVE LICENSE APPLICATIONS <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill <br />and carried, the following license applications were approved <br />and referred to the Deputy Controller's office for issuance: <br />Secondhand Dealer - Neat Stuff (Marla Dill) <br />804 W. Indiana, S.B. <br />Public Parking - National Autopark, Inc. <br />119 W. Wayne, S.B. <br />Open Air Stand - Cambodia's Egg Rolls <br />135 S. Michigan (Plaza) <br />Rubbish & Garbage - Sylvester Mullins <br />115 S. Cherry, S.B. <br />Grayson Warren <br />1922 N. Meade, S.B. <br />Superior Waste Systems, Inc. <br />54107 Butternut, S.B. <br />Additionally, upon a motion made by Mr. Leszczynski, seconded by <br />Mr. Hill and carried, the massage technician renewal application <br />received by the Board for approval was referred back to the <br />Controller's Office for issuance, due to the fact that the <br />Massage Technician License is not subject to approval by the <br />Board of Public Works. <br />1 <br />1 <br />1 <br />
