REGULAR MEETING JULY 2, 1984 26 APPROVE BUREAU OF HOUSING NEIGHBORHOOD PAINT -UP PROGRAM AGREEMENT Ms. Janet S. Holston, Director, Bureau of Housing, submitted to the Board for approval, one (1) Agreement for Rehabilitation of Real Property between the Board of Public Works and Home- owner and Agreement for the Repair of Real Property between the Board of Public Works and Contractor as follows: HOMEOWNER: Gilbert/Pat Williams ADDRESS: 1111 Stanfield CONTRACTOR: B & C Construction Company CONTRACT AMOUNT: $473.00 Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, the above Agreement was approved and executed. APPROVE CHANGE ORDERS NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVITS - MIAMI/IRELAND RETENTION BASIN REPAIRS; CREST MANOR RETENTION POND REPAIRS PROJECT AND JEFFERSON STREET PARKING GARAGE CONCRETE REPAIRS Mr. Leszczynski advised that the following Change Orders No. 1 (Final) and Project Completion Affidavits have been received: MIAMI/IRELAND RETENTION BASIN REPAIRS Contractor: L. Wolff Farms, Inc. 2017 W. 1000.S. Union Mills, Indiana Increase: $2,154.04 New Contract Sum: $44,667.04 CREST MANOR RETENTION POND REPAIRS PROJECT Contractor Eck Construction 4320 Cottage Avenue Mishawaka, Indiana Increase: $19,779.00 New Contract Sum: $40,879.00 JEFFERSON STREET PARKING GARAGE CONCRETE REPAIRS Contractor: William Hach and Associates, Inc. 220 Deerpath Road Bensonville, Illinois Increase: $34,196.89 New Contract Sum: $170,820.14 Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and carried, the above Change Orders No. 1 (Final) and Project Completion Affidavits indicating the new final costs were approved subject to the filing of the appropriate three-year Maintenance Bonds. APPROVE REQUEST TO HAVE THE 500 BLOCK OF PENNSYLVANIA AVENUE CLOSED FOR FAMILY REUNION (JULY 4, 1984) In a letter to the Board, Mr. Algerine Henderson, 520 East, Pennsylvania Avenue, requested that the 500 block of Pennsylvania Avenue be closed from Niedsel Street to Rush Street east from 1:00 p.m. to 8:00 p.m. on July 4, 1984 for the purpose of a family reunion. It was noted that the letter contained signatures. of three (3) residents of the neighborhood indicating that they had no objections to the street being closed. Mr. Leszczynski advised that Mr. Ralph J. Wadzinski, Manager, Bureau of Traffic and Lighting, has reviewed the above request and recommends approval subject to the maintenance of a ten (10) foot clear and unobstructed lane for emergency purposes. It was noted that the Petitioner will be responsible for the closing and opening of the street and alleys with barricades to be provided by the Bureau of Traffic and Lighting. Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, the above request was approved. i
