Holston, Director, Bureau of Housing, submitted to <br />the Board for approval, one (1) Agreement for Rehabilitation <br />of Real Property between the Board of Public Works and Home- <br />owner and Agreement for the Repair of Real Property between <br />the Board of Public Works and Contractor as follows: <br />HOMEOWNER: Gilbert/Pat Williams <br />ADDRESS: 1111 Stanfield <br />CONTRACTOR: B & C Construction Company <br />CONTRACT AMOUNT: $473.00 <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill <br />and carried, the above Agreement was approved and executed. <br />APPROVE CHANGE ORDERS NO. 1 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVITS - MIAMI/IRELAND RETENTION BASIN REPAIRS; CREST <br />MANOR RETENTION POND REPAIRS PROJECT AND JEFFERSON STREET <br />PARKING GARAGE CONCRETE REPAIRS <br />Mr. Leszczynski advised that the following Change Orders No. 1 <br />(Final) and Project Completion Affidavits have been received: <br />MIAMI/IRELAND RETENTION BASIN REPAIRS <br />Contractor: L. Wolff Farms, Inc. <br />2017 W. 1000.S. <br />Union Mills, Indiana <br />Increase: $2,154.04 <br />New Contract Sum: $44,667.04 <br />CREST MANOR RETENTION POND REPAIRS PROJECT <br />Contractor Eck Construction <br />4320 Cottage Avenue <br />Mishawaka, Indiana <br />Increase: $19,779.00 <br />New Contract Sum: $40,879.00 <br />JEFFERSON STREET PARKING GARAGE CONCRETE REPAIRS <br />Contractor: William Hach and Associates, Inc. <br />220 Deerpath Road <br />Bensonville, Illinois <br />Increase: $34,196.89 <br />New Contract Sum: $170,820.14 <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and <br />carried, the above Change Orders No. 1 (Final) and Project <br />Completion Affidavits indicating the new final costs were approved <br />subject to the filing of the appropriate three-year Maintenance <br />Bonds. <br />APPROVE REQUEST TO HAVE THE 500 BLOCK OF PENNSYLVANIA <br />AVENUE CLOSED FOR FAMILY REUNION (JULY 4, 1984) <br />In a letter to the Board, Mr. Algerine Henderson, 520 East, <br />Pennsylvania Avenue, requested that the 500 block of Pennsylvania <br />Avenue be closed from Niedsel Street to Rush Street east from <br />1:00 p.m. to 8:00 p.m. on July 4, 1984 for the purpose of a <br />family reunion. It was noted that the letter contained signatures. <br />of three (3) residents of the neighborhood indicating that they <br />had no objections to the street being closed. Mr. Leszczynski <br />advised that Mr. Ralph J. Wadzinski, Manager, Bureau of Traffic <br />and Lighting, has reviewed the above request and recommends <br />approval subject to the maintenance of a ten (10) foot clear <br />and unobstructed lane for emergency purposes. It was noted <br />that the Petitioner will be responsible for the closing and <br />opening of the street and alleys with barricades to be provided <br />by the Bureau of Traffic and Lighting. Upon a motion made by <br />Mr. Leszczynski, seconded by Mr. Hill and carried, the above <br />request was approved. <br />i <br />
