(Requested <br />by Bureau of Traffic & Lighting). <br />APPROVE HANDICAPPED PARKING PERMITS <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and <br />carried, the following four (4) handicapped parking permit <br />applications were approved and referred to the Deputy Controller's <br />office for issuance of permits <br />1. Noel Wolford, 729 E. Dubail, S.B. <br />2. Edward P. Leahy, Sr., 519 E. Dubail, S.B. <br />3. Mary Sweeney, 112 Niles, S.B. <br />4. Julius Hawes, 53363 Wallis Dr., S.B. <br />FILING OF HUMANE SOCIETY REPORT - MAY <br />The Humane Society Report for the month of May indicated the <br />following matters handled for the City of South Bend: <br />DOGS .HANDLED: 285 <br />CATS HANDLED: 201 <br />OTHER ANIMALS HANDLED: 133 <br />BIT CASES HANDLED: 13 <br />COMPLAINTS INVESTIGATED: 207 <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and <br />carried, the above request was accepted and filed. <br />FILING OF CERTIFICATE OF INSURANCE <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and <br />carried, the Certificate of Insurance for Sollitt Construction <br />Company, Inc., P.O. Box 87, South Bend, Indiana, was accepted <br />and filed. <br />APPROVE CLAIMS <br />Mr. Elroy J. Kelzenberg, Executive Director, St. Joseph County <br />Job Training Program, submitted a list from the Michiana Area <br />CETA Consortium containing three (3) claims and recommended <br />approval. Mr. Kelzenberg additionally submitted two lists <br />from the Job Training Program containing eight -five (85) <br />claims and recommended approval. <br />City Controller Michael L. Vance submitted Claim Docket No. <br />11076 through Claim Docket No. 11933 and recommended approval. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and <br />carried, the above claims were approved"and the reports were <br />filed. <br />APPROVE ADDENDUM TO LEASE AGREEMENT BETWEEN THE CITY <br />AND EDMARDE'S, INC. D/B/A SENOR KELLY'S FOR A <br />PORTION OF ALLEN'S ALLEY <br />Carolyn V. Pfotenhauer, Assistant City Attorney, submitted to <br />the Board for approval an Addendum to the Lease Agreement <br />between the City and Edmarde's, Inc., d/b/a Senor Kelly's <br />originally entered into on October 4, 1982, for the lease of <br />a portion of Allen's Alley more particularly described in the <br />legal description contained in the Addendum.
