REGULAR MEETING DECEMBER 12, 1983 APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - MAPLE ROAD INTERSECTION TURN `LANES ON'S.R. 23 (RITSCHARD BROS., INC.) Mr. Leszczynski advised that Ritschard Bros.,.Inc. has submitted Change Order No. 1 (Final) indicating that the contract amount be increased by $26,439.37 for a new contract sum including this Change Order in the amount of $101,627.66. Additionally submitted was the Project Completion Affidavit indicating, this, new final. cost-. Upon a moti:on.made by Mr. Leszczynski, seconded by Mr. Hill and .carried, Change Order No. 1'(Final) and the Project Completion Affidavit were approved and the' -appropriate three-year MaintenanceBondwas filed. APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - SIGNALIZATION'OF ORANGE STREET & COLLEGE STREET AND COLFAX AVENUE AND WALNUT STREET INTERSECTIONS (TRANS TECH ELECTRIC, INC.) Mr. Leszczynski advised that Trans Tech Electric, Inc. has submitted Change Order No. 1 (Final) indicating that the contract amount be increased by $3,274.30 for a new contract sum including this Change Order in the amount of $50,723.50. Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved subject to the filing of the appropriate three-year Maintenance Bond. APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - CONCRETE PAVEMENT REPAIRS, VARIOUS LOCATIONS 1983 (THE ROBERT HENRY CORPORATION) Mr. Leszczynski advised that The Robert Henry Corporation has submitted Change Order No. 1 (Final) indicating that the contract amount be increased by $22,406.94 for a new contract sum including this Change Order in the amount of $96,014.44. Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved and the appropriate three-year Maintenance Bond was filed. APPROVE STATE OF INDIANA DISTRICT ENGINEER'S REPORT OF INSPECTION AND RECOMMENDATION FOR INSPECTION (COLFAX AVENUE BRIDGE PROJECT) Mr. Leszczynski presented to the Board the State of Indiana District Engineer's Report of Inspection and Recommendation for Acceptance concerning the Colfax Avenue Bridge Project indicating that preliminary and final inspections of the bridge were made and the date of last work of Contractor was November 28, 1983. Upon a motion made by Mr. Leszczynski, seconded by Mr. Kernan and carried, the above referred to document was approved and executed. APPROVE REQUEST TO CONDUCT .'SAVE OUR JOBS" PUBLIC MEETING ON RIVER BEND PLAZA (DECEMBER 17, 1983) Mr. Leszczynski advised that Ms. Mikki Dobski, City Events Coordinator, has reviewed the request of Mr. Michael Matuszak, presented to the Board on November 28, 1983 to conduct a public hearing on River Bend Plaza regarding plant closings on December 17, 1983 and recommends approval. Upon a motion made by Mr. Leszczynski, seconded by Mr..Hill and carried, the above request was approved subject to Mr. Matuszak executing an Indemnification Agreement holding the City harmless from any liability resulting from this event.
