REGULAR MEETING DECEMBER 10, 1985 Upon a motion made by and carried, the above of Action was accepted Mr. Leszczynski, seconded by Ms. Barnard referenced Resolution and Official Notice and approved. APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - RECONSTRUCTION OF FIRE STATION NO. 7 (1616 PORTAGE) Mr. Leszczynski advised that Kaser-Spraker Construction, 25487 West State Road 2, P.O. Box 3605. South Bend, Indiana, has submitted Change Order No. 1 (Final) indicating that the Contract amount be increased by $2,460.00 for a new Contract sum including this Change Order in the amount of $45,192.00. It was noted that the Change Order indicates changes in four (4) Contract items. Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Leszczynski, seconded by Ms. Barnard and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved subject to the filing of the appropriate three-year Maintenance Bond. APPROVE REQUEST TO ERECT CANOPY - MERSITS MARKET, 2821 L.W.W. Mr. Leszczynski advised that the Bureau of Traffic and Lighting and the Building Department have reviewed the above referred to request, submitted to the Board on November 26, 1985 and recommend approval subject to the following: - As L.W.W. is a state highway, Mersits Market must secure the appropriate approval from the Indiana Department of Highways before a permit to erect can be obtained from the Building Department. As the canopy will encroach over the public right-of- way, Mersits Market must provide the appropriate $1,000,000.00 perpetual Certificate of Insurance naming the City of South Bend as an additional insured. - Installation of the canopy must meet all city ordinance requirements and drawings must be submitted to the Building Department for review prior to installation. Upon a motion made by Mr. Leszczynski, seconded by Ms. Barnard and carried, the request was approved subject to the above referred to requirements being met. FILING OF ENVIRONMENTAL CLEAN-UP OF LOTS REPORT A report from the Department of Code Enforcement indicating a total of sixteen (16) properties cleaned from December 2, 1985 to December 5, 1985, was submitted. Upon a motion made by Mr. Leszczynski, seconded by Ms. Barnard and carried, the report as submitted was filed. FILING OF STREET LIGHT OUTAGE REPORT - NOVEMBER The report submitted to the Board indicated a total of one -hundred fifty-eight (158) street light outages reported during the month of November. Upon a motion made by Mr. Leszczynski, seconded by Ms. Barnard and carried, the report as submitted was filed. APPROVE CLAIMS Deputy City Controller Carol M. Sanders submitted Claim Docket No. 27183 through Claim Docket No. 27641 and recommended approval. Upon a motion made by Ms. Barnard, seconded by Mr. Leszczynski carried, the claims were approved and the report filed.
