REGULAR MEETING <br />DECEMBER 10, 1985 <br />Upon a motion made by <br />and carried, the above <br />of Action was accepted <br />Mr. Leszczynski, seconded by Ms. Barnard <br />referenced Resolution and Official Notice <br />and approved. <br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVIT - RECONSTRUCTION OF FIRE STATION NO. 7 (1616 PORTAGE) <br />Mr. Leszczynski advised that Kaser-Spraker Construction, 25487 <br />West State Road 2, P.O. Box 3605. South Bend, Indiana, has <br />submitted Change Order No. 1 (Final) indicating that the Contract <br />amount be increased by $2,460.00 for a new Contract sum including <br />this Change Order in the amount of $45,192.00. It was noted that <br />the Change Order indicates changes in four (4) Contract items. <br />Additionally submitted was the Project Completion Affidavit <br />indicating this new final cost. Upon a motion made by Mr. <br />Leszczynski, seconded by Ms. Barnard and carried, Change Order <br />No. 1 (Final) and the Project Completion Affidavit were approved <br />subject to the filing of the appropriate three-year Maintenance <br />Bond. <br />APPROVE REQUEST TO ERECT CANOPY - MERSITS MARKET, 2821 L.W.W. <br />Mr. Leszczynski advised that the Bureau of Traffic and Lighting <br />and the Building Department have reviewed the above referred to <br />request, submitted to the Board on November 26, 1985 and <br />recommend approval subject to the following: <br />- As L.W.W. is a state highway, Mersits Market must <br />secure the appropriate approval from the Indiana <br />Department of Highways before a permit to erect can <br />be obtained from the Building Department. <br />As the canopy will encroach over the public right-of- <br />way, Mersits Market must provide the appropriate <br />$1,000,000.00 perpetual Certificate of Insurance naming the <br />City of South Bend as an additional insured. <br />- Installation of the canopy must meet all city ordinance <br />requirements and drawings must be submitted to the Building <br />Department for review prior to installation. <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Barnard <br />and carried, the request was approved subject to the above <br />referred to requirements being met. <br />FILING OF ENVIRONMENTAL CLEAN-UP OF LOTS REPORT <br />A report from the Department of Code Enforcement indicating a <br />total of sixteen (16) properties cleaned from December 2, 1985 to <br />December 5, 1985, was submitted. Upon a motion made by Mr. <br />Leszczynski, seconded by Ms. Barnard and carried, the report as <br />submitted was filed. <br />FILING OF STREET LIGHT OUTAGE REPORT - NOVEMBER <br />The report submitted to the Board indicated a total of <br />one -hundred fifty-eight (158) street light outages reported <br />during the month of November. Upon a motion made by Mr. <br />Leszczynski, seconded by Ms. Barnard and carried, the report as <br />submitted was filed. <br />APPROVE CLAIMS <br />Deputy City Controller Carol M. Sanders submitted Claim Docket <br />No. 27183 through Claim Docket No. 27641 and recommended <br />approval. Upon a motion made by Ms. Barnard, seconded by Mr. <br />Leszczynski carried, the claims were approved and the report <br />filed. <br />
