Continued EXHIBIT "C" <br />WORK ACTIVITY REPORT FIELD DESCRIPTION <br />A WORK ACTIVITY REPORT is to be completed for all work performed on traffic signals. <br />Procedure for completing "Work Activity Report" form. <br />1. REPORT NO: Activity report number. <br />Format -month year-DataID-report number: 0102-3098-001 (Office Use only) <br />2. WORK ORDER NO.- If work being done is for a Work Order then enter the work order number. <br />3. COMM NO.- Commission number given on the intersection list. This number is on the cards in <br />the cabinet, prints and timing sheets as well as listed in the intersection list. <br />4. LOCATION- Intersection name. This should always include the State, US or Interstate. <br />5. CITY- The CITY where work is being done. <br />6. REPORTED BY- Who reported the trouble call and their agency (if applicable) <br />7. ACTIVITY- This designates whether this work is E: Emergency or S: (Non -Routine, Work order <br />or Scheduled work) <br />8. ACCIDENT- Indicate if an accident occurred at any time while at the intersection. <br />9. TIME RECV- The time this problem was reported in military time. <br />10. DATE RECV- The date the problem was reported. <br />11. TIME ARRV- Time arrived at the problem location in military time <br />12. DATE ARRV- The date arrived at location. <br />13. TIME COMP- Time completed in military time. <br />14. DATE COMP- The date completed. <br />15. PROB REP 1 & 2- The malfunction reported and the appropriate code from the "Reported <br />Problem" code list. Up to 2 (two) problems can be reported. <br />16. PROB FD 1, 2, 3, & 4- The observed problem again from the "Reported Problem" code list. Up to <br />4 (FOUR) malfunctions can be recorded. <br />17. ACTION 1, 2, 3, 4, 5 and 6- Results of the job and the appropriate code number from "TROUBLE <br />CALL REPAIR CODES". Up to 6 (SIX) actions. <br />18. COMMENTS- Each specific item (loop, head, span, junction box, pole, detector housing, ...) on <br />which work was performed shall be identified as to location and work performed in the comment <br />section of The Work Activity Report. <br />19. LABOR - <br />A. EMPLOYEE ID- All personnel involved with doing work for this report. First four letters of <br />last name and first letter of first name. <br />B. HOURS- Number of hours each employee worked on this report. <br />20. MATERIAL - <br />A. TRANSFER TYPE- Install, Remove, Receive, Return of material. <br />B. INVENTORY NO: Department number, assigned to specific material. <br />C. MODEL NO.- Specific model number of material. (Control Equipment) <br />D. SERIAL NO.- Serial number of material (Control Equipment) <br />E. QUANTITY- Number of each INVENTORY NO. transferred. <br />21. EQUIPMENT - <br />A. EQUIPMENT- All equipment used on this report. <br />B. HOURS- Number of hours that equipment was used on this report. <br />Page 34 of 53 Pages <br />
