EXHIBIT "B" <br />PREVENTIVE MAINTENANCE CHECK LIST <br />1. Signal Indications: to be performed on the periodic scheduled visits biannually. <br />A. All lights operating <br />B. In proper sequence <br />C. No lenses broken or cracked <br />2. Signal Heads: to be checked in conjunction with the `annual signal bulb change -out, lens cleaning and <br />aerial inspection' scheduled visit. <br />A. Proper height (visual check) <br />B. Proper Alignment: Horizontal, vertical, and rotation of all types of heads including a check of the <br />aiming of programmable heads. Aligning of signal heads will not be permitted unless the <br />appropriate set screw, nut, bolt, or screw is also tightened. A tool appropriate for tightening shall <br />be used for aligning signal heads, replacing lenses, replacing visors, or making adjustments. <br />C. No visors missing. Visors or louvers which are missing or damaged shall be replaced. The District <br />Traffic Engineer shall be contacted for availability of replacement visors. <br />D. No pinnacles missing <br />E. Annual check of eye bolt, weatherhead, and balance adjuster- to be done with annual bulb <br />change/ inspection/ cleaning <br />F. Terminal block connections snug (in disconnect hangers, if present) to be done with annual bulb <br />change/inspection/cleaning <br />G. Check for snugness of all mechanical connections - annual <br />H. Wires shall not be closed in doors. Sockets shall not be tightened into terminal strips. Lamps shall <br />not be over tightened or under tightened. <br />3. Overhead Cables: to be checked in conjunction with the "annual signal bulb change -out, lens cleaning <br />and aerial inspection" scheduled visit. <br />A. Correct drip loops: loops should not allow water to flow into signal heads or disconnect hangers. <br />B. No improper splices and visual inspection of splices, repair, or damaged splices. <br />C. Right number of cable clamps - annually <br />D. Proper spacing of cable rings - annually <br />E. No sagging or loose cables or "A" wires. <br />F. Down guys tight <br />G. Pole eye bolts in good condition - annually <br />H. Check for worn wires at weatherhead, to be done at annual bulb change/inspection/cleaning <br />4. Service Disconnect Box: to be checked in conjunction with the "annual signal bulb change -out, lens <br />cleaning and aerial inspection" scheduled visit. <br />A. Box and conduit mounted securely <br />B. All connections snug <br />C. Ground wire secured to pole <br />D. Ground rod clamp snug - if possible <br />E.
