<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 6 on behalf
<br />of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614,
<br />indicating the Contract amount be increased by $363,165.83 for a new contract stun, including
<br />this Change Order, in the amount of $16,954,944.70. Upon a motion made by Mr. Relos,
<br />seconded by Ms. Maradik and carried, the Change Order was approved.
<br />APPROVE CIIANGE ORDER NO. 7 — SOUTH BEND ONE WAY TO TWO WAY
<br />CONVERSION, DIVISION B — PROJECT NO. 116-001 (TIF BOND)
<br />Mr. Oiler advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 7 on behalf —.
<br />of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614,
<br />indicating the Contract amount be increased by $293,073.61 for a new contract sum, including
<br />this Change Order, in the amount of $17,248,018.31. Upon a motion made by Mr. Relos,
<br />seconded by Ms. Maradik and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— WASTEWATER TREATMENT PLANT BELT FILTER PRESS NO. 2 REIIABILITATION
<br />— PROJECT NO 115-082 (ENVIRONMENTAL SERVICES AND WASTEWATER
<br />MAINTENANCE AND REPAIRS)
<br />Mr. Gilot advised that Mr. Jacob Klosinski, Environmental Services, has submitted Change
<br />Order No. 1 (Final) on behalf of Alfa Laval, Inc., 10470 Deer Trail Drive, Houston, Texas
<br />77038, indicating the contract amount be decreased by $5,000.00 for a new contract sum,
<br />including this Change Order, of $193,087.00. Additionally submitted was the Project
<br />Completion Affidavit indicating this new final cost of $183,087.00. Upon a motion made by Mr.
<br />Relos, seconded by Ms. Maradik and carried, Change Order No. I (Final) and the Project
<br />Completion Affidavit were approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final) on
<br />behalf of Martell Electric, LLC, 46601 Cleveland Road, South Bend, Indiana 46628, indicating
<br />The contract amount be decreased by $7,193.05 for a new contract sum, including this Change
<br />Order, of $79,171.20. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $79,171,20, Upon a motion made by Ms. Maradik, seconded by Mr. Relos
<br />and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 3 (Final) on
<br />behalf of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, Indiana 46614, indicating the
<br />contract amount be decreased by $5,190.91 for a new contract sum, including this Change Order,
<br />of $182,715.29, Additionally submitted was the Project Completion Affidavit indicating this
<br />new final cost of $182,715 29. Upon a motion made by Ms. Maradik, seconded by Mr. Relos
<br />and carried, Change Order No. 3 (Final) and the Project Completion Affidavit were approved.

<br />In a memorandum to the Board, Mr. Richard Estes, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Therefore, upon a motion made by Mr. Relos,
<br />seconded by Ms. Maradik and carried, the above request to advertise was approved, and the Title
<br />Sheet was approved and signed.
