Jefferson Blvd. <br />South Bend, IN 46601 <br />The St. Joseph Cowity Health Department (SJCHD) is applying to the Indiana State <br />Department of Health (ISDH) to be considered on their application for the <br />Communities Putting Prevention to Work funding opportunity through the American <br />Recovery and Reinvestment Act of 2009 (FO# CDC-RFA-DP09-912ARRA09, CFDA# <br />93.724). The Recovery Act includes $650 million for evidence-based clinical and <br />community-based prevention and wellness strategies to reduce chronic disease rates, <br />with $1._5-4 million potentially available for our area. ISDH is eligible to apply on <br />behalf of two small cities/rural areas. We would like to be included on State's <br />application in order to bring these potential funds to South Bend. <br />In order to be considered by the State as one of the two cities, we need to prove that we <br />are a highly qualified applicant with an extensive support network in place. We are not <br />yet writing the grant; we are only applying to State to be considered for their <br />application. In our Letter of Intent (LOI) due to State on October 20, 2009, we must <br />include brief, yet official notes in support of the potential application from city planners <br />and developers. <br />We would Greatly appreciate a quick note to show your support of our application to <br />State. An official Letter of Support may be required if we are accepted as one of the <br />two applicants.
