Klosinski, Assistant City Engineer SUBJECT: Agenda Request Form — Request to Advertise Project No. 115-082 WWTP Belt Filter Press Rehabilitation DATE: September 14, 2015 The Division of Utilities is submitting Change Order No. 1 and the Project Completion Affidavit for Project No. 115-082 WWTP Belt Filter Press #2 Rehabilitation to the Board of Public Works for review and approval at the September 27, 2017 Board of Public Works Meeting. Alfa Laval has completed all work on Belt Filter Press #2. The $5,000 allowance for unforeseen issues and miscellaneous repairs was not needed during the course of the work. Therefore, Change Order No. 1 addresses this as a credit back to the City of South Bend since the funds were not used during construction. The result is a -2.65% decrease to the final contract price for the work. The funding source for this contract will be Utilities, Division of Environmental Services, Wastewater Repair and Maintenance Other Equipment (641-0630-793-36-05).
