262 <br />REGULAR MEETING <br />AUGUST 28, 1989 <br />$18,778.22 for a new Contract sum including this Change Order in <br />the amount of $72,175.22. Additionally submitted was the Project <br />Completion Affidavit indicating this new final cost. Mr. <br />Leszczynski noted that this Change.Order reflects items including <br />rotomilling and using sodding instead of seeding. <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys <br />and carried, Change Order No. 1 (Final) and the Project <br />Completion Affidavit were approved and the appropriate three-year <br />Maintenance Bond was accepted for filing. <br />APPROVE TITLE SHEET - FARMINGTON SQUARE, UNIT 13 <br />Mr. Leszczynski advised that the Title Sheet for the above <br />referred to project which indicates sanitary sewer and water was <br />being presented at this time for execution. Upon a motion made <br />by Mr. Leszczynski, seconded by Ms. Humphreys and carried, the <br />above referred to Title Sheet was approved and signed. <br />APPROVE AND/OR RELEASE CONTRACTORS AND EXCAVATION BONDS <br />Mr. Michael P. Meeks, Director, Division of Engineering, <br />recommended that the following Contractors and Excavation Bonds <br />be approved and/or released as indicated below: <br />CONTRACTORS BONDS <br />McCollough-Scholten Construction, Inc. Approved <br />Effective: August 28, 1989 <br />Steve's Concrete, Inc. Released <br />Effective: August 21, 1989 <br />EXCAVATION BOND <br />McCollough-Scholten Construction, Inc. Approved <br />Effective: August 28, 1989 <br />FILING OF CERTIFICATE OF INSURANCE <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys <br />and carried, the Certificate of Insurance for Edward J. White, <br />Inc., P.O. Box 898, South Bend, Indiana, was accepted and filed. <br />FILING OF ENVIRONMENTAL CLEAN-UP OF LOTS REPORT <br />A report from the Department of Code Enforcement indicating a <br />total of eighty-two (82) properties cleaned from August 7, 1989 <br />to August 16, 1989, was submitted. Upon a motion made by Mrs. <br />DeClercq, seconded by Ms. Humphreys and carried, the report as <br />submitted was filed. <br />APPROVE CLAIMS <br />Mr. Juan Manigault, Executive Director, St. Joseph County Job <br />Training Program submitted a list containing forty-two (42) <br />claims and recommended approval. <br />Additionally, Chief Deputy Controller John D. Leisenring, <br />submitted Claim Docket No. 13323 through Claim Docket No. 13638 <br />and recommended approval. Upon a motion made by Ms. Humphreys, <br />seconded by Mrs. DeClercq and carried, the claims were approved <br />and the reports as submitted were filed. <br />1 <br />I <br />
