REGULAR MEETING AUGUST 7, L989 <br />227 <br />1 <br />J <br />The regular meeting of the Board of Public Works was convened at <br />9:35 a.m. on Monday, August 7, 1989 by Board President John E. <br />Leszczynski, with Mr. Leszczynski, Mrs. Patricia E. DeClercq and <br />Ms. Katherine Humphreys present. Also present was Assistant City <br />Attorney Jenny Pitts-Manier. <br />APPROVE MINUTES OF PREVIOUS MEETING <br />Upon a motion made by Mrs. DeClercq, seconded by Ms. Humphreys <br />and carried, the minutes of the regular meeting of the Board held <br />on July 31, 1989 were approved. <br />AGENDA ITEM ADDED <br />Upon a motion made by Ms. Humphreys, seconded by Mrs. DeClercq <br />and carried, an Excavation Bond was added to the agenda. <br />AGENDA ITEM STRICKEN <br />There being no objections by members of the Board, a Contract for <br />Staff Support/Business Development Corporation was stricken from <br />the agenda. <br />OPENING OF PROPOSALS - EMPLOYEE ASSISTANCE PROGRAM SERVICES AND <br />FILING OF APPLICATIONS FOR BIDDER QUALIFICATION <br />Mr. Leszczynski advised that this was the date set for the <br />receiving and opening of sealed Proposals for the above referred <br />to services. The Clerk tendered proofs of publication of Notice <br />in the South Bend Tribune and the Tri-County News which were <br />found to be sufficient. Proposals from the following were <br />received by the Board for Employee Assistance Program Services: <br />MADISON CENTER <br />403 East Madison Street <br />South Bend, Indiana 46617 <br />Submitted by Susan K. Carter, Director of Marketing and Lori <br />Lohman, Marketing Representative for Business & Industry <br />Services. <br />PROPOSAL: <br />One twelfth (1/12) of the flat fee of: <br />Total: <br />FAMILY & CHILDREN'S SERVICES. INC. <br />1411 Lincoln Way West <br />Mishawaka, Indiana 46544-1690 <br />Submitted by Thomas B., <br />Vice-President/C.O.O. <br />PROPOSAL: <br />Annual Cost based on employee size <br />of 1150: <br />Annual EAP (Estimated): <br />MICHIANA EMPLOYEE ASSISTANCE PROGRAM <br />Jefferson Square, 818 E.
