CLAIMS REViFW MEETING SEPTEMBER 20 2016 296 <br />A Claims Review Meeting of the Board of Public Works was convened at 9:01 a.m. on Tuesday, <br />September 20, 2016, by Board Member David Relos with Board Members Elizabeth Maradik and <br />,lames Mueller present, Board President Gary Gilot and Board Member Therese Doran were <br />absent. The meeting was held in the 13"' Floor Boardroom, County -City Building, 227 W. <br />Jefferson Blvd., South Bend, Indiana. <br />APPROVE CLAIMS <br />Ms. Maradik stated that a request to pay claims has been received by the Board; each claim is fully <br />supportedby an invoice; the person receiving the goods or services has approved the claims; and <br />the claims have been filed with the City Fiscal Officer and certified for accuracy. Ms. Maradik <br />stated the followin claims were submitted to the Board for approval: _ <br />Name Amount of Claim Date <br />City of South Bend $2 168 559.96 09/20/2016 <br />U. S. Bank National Association, Requisition No. 45; Smart $4,998.75 09/16/2016 <br />Streets Bond <br />therefore, Ms. Maradik made a motion that —after-- review of the expenditures, the claims be <br />approved as submitted.
