The price for Wood Floor Removal and Disposal as <br />Special Waste. <br />2. Bierlein's intent is to ship Special Waste to Prairie <br />View Landfill only. <br />3. The Special Waste prices are subject to review and <br />approval of the waste material at the respective sites <br />and State permitting at Prairie View Landfill. <br />4. The City/Owner has all information needed to obtain <br />the Solid Waste Permits from the Indiana Department of <br />Environmental Management, Solid and Hazardous Waste <br />Section for the wood flooring material. <br />5. Bierlein has included only two TCLP tests for the <br />acceptance of wood flooring disposal at Prairie View <br />Landfill. Any other costs of analysis for acceptance <br />by the disposal facility or (IDEM), (EPA) are not <br />included. <br />6. Timing of demolition will start after all permits and <br />licenses have been obtained and asbestos abatement is <br />completed. <br />ADAMO DEMOLITION COMPANY, INC. <br />300 East Seven Mile Road <br />Detroit, Michigan 48203 <br />Bid was signed by Mr. John T. Adamo, President, <br />Non -Collusion Affidavit was in order, Non -Discrimination <br />Commitment form was completed and a five per cent (5%) <br />Bid Bond was submitted. <br />BID PACKAGE NO. 2 BASE BID PRICE: $1,269,231.50 <br />Board Attorney Jenny Pitts Manier advised that the bids submitted <br />by Dore & Associates Contracting, 900 Harry S. Truman Parkway, <br />P.O. Box 146, Bay City, MI and Zebrowski & Associates, Inc., P.O. <br />Box 391, Farmington, Michigan, did not contain the notary seal in <br />the notary jurat. She noted that the bids were properly signed <br />by the bidders and the respective notary but the seal was missing <br />in both instances. Ms. Manier further noted that the purpose of <br />the notary is to verify that the person signing the bid is who <br />they say they are. It was also noted that the Board could accept <br />and read these bids and reject them later should they find them <br />non -responsive. <br />Therefore, Mr. Leszczynski made a motion that the bids of Dore & <br />Associates Contracting and Zebrowski & Associates, Inc., be <br />publicly read and that the persons signing the bid forms be <br />contacted to confirm their signatures. Ms. Humphreys seconded <br />the motion which carried and the following bids were read: <br />ZEBROWSKI & ASSOCIATES INC. <br />P.O.
