REGULAR MEETING DECEMBER 31. 1990 423 1 1 BIERLEIN DEMOLITION CONTRACTORS, INC. 2903 South Graham Road Saginaw, MI 48603 Bid was signed by Mr. Michael Bierlein, President, Non -Collusion Affidavit was in order, Non -Discrimination Commitment form was completed and a five per cent (5%) Bid Bond was submitted. BID PACKAGE NO. 2 BASE BID PRICE: $ 987,654.32 Qualifications: 1. The price for Wood Floor Removal and Disposal as Special Waste. 2. Bierlein's intent is to ship Special Waste to Prairie View Landfill only. 3. The Special Waste prices are subject to review and approval of the waste material at the respective sites and State permitting at Prairie View Landfill. 4. The City/Owner has all information needed to obtain the Solid Waste Permits from the Indiana Department of Environmental Management, Solid and Hazardous Waste Section for the wood flooring material. 5. Bierlein has included only two TCLP tests for the acceptance of wood flooring disposal at Prairie View Landfill. Any other costs of analysis for acceptance by the disposal facility or (IDEM), (EPA) are not included. 6. Timing of demolition will start after all permits and licenses have been obtained and asbestos abatement is completed. ADAMO DEMOLITION COMPANY, INC. 300 East Seven Mile Road Detroit, Michigan 48203 Bid was signed by Mr. John T. Adamo, President, Non -Collusion Affidavit was in order, Non -Discrimination Commitment form was completed and a five per cent (5%) Bid Bond was submitted. BID PACKAGE NO. 2 BASE BID PRICE: $1,269,231.50 Board Attorney Jenny Pitts Manier advised that the bids submitted by Dore & Associates Contracting, 900 Harry S. Truman Parkway, P.O. Box 146, Bay City, MI and Zebrowski & Associates, Inc., P.O. Box 391, Farmington, Michigan, did not contain the notary seal in the notary jurat. She noted that the bids were properly signed by the bidders and the respective notary but the seal was missing in both instances. Ms. Manier further noted that the purpose of the notary is to verify that the person signing the bid is who they say they are. It was also noted that the Board could accept and read these bids and reject them later should they find them non -responsive. Therefore, Mr. Leszczynski made a motion that the bids of Dore & Associates Contracting and Zebrowski & Associates, Inc., be publicly read and that the persons signing the bid forms be contacted to confirm their signatures. Ms. Humphreys seconded the motion which carried and the following bids were read: ZEBROWSKI & ASSOCIATES INC. P.O. Box 391 Farmington, Michigan 48024 Bid was signed by Mr. Edward Tim Sullivan, President, Non -Collusion Affidavit was in order, Non -Discrimination Commitment form was completed and a five per cent (5%) Bid Bond was submitted.
