Parmerlee, Clerk APPROVE CHANGE ORDER NO. 3 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - VIEWING PARK PROJECT Mr. Leszczynski stated that Kaser-Spraker Construction, Inc., 25487 West State Road 2, P.O. Box 3605, South Bend, Indiana, has submitted Change Order No. 3 (Final) indicating that the Contract amount be decreased by $7,755.00 for a new Contract sum including this Change Order in the amount of $304,623.00. Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys and carried, Change Order No. 3 (Final) and the Project Completion Affidavit were approved and the appropriate three-year Maintenance Bond as submitted was accepted for filing. APPROVE PROJECT COMPLETION AFFIDAVIT - ST. JOSEPH/WAYNE STREET PARKING GARAGE PEDESTRIAN WALKWAY PROJECT Mr. Leszczynski indicated that Murdock & Sons, 1320 South Franklin Street, South Bend, Indiana, has submitted the Project Completion Affidavit for the above referred to project which indicates a final cost of $165,549.00. Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and carried, the Project Completion Affidavit was approved'and the required three-year Maintenance Bond as submitted was accepted for filing. ACCEPTANCE OF CASHIER'S CHECK REPRESENTING AMOUNT OF UNCOMPLETED PUBLIC IMPROVEMENTS - MAYFLOWER 6TH ADDITION Mr. Leszczynski advised that the Engineering Department has submitted to the Board Cashiers Check #01-55250 in the amount of $500.00 as received from Mr. Dave Coker in lieu of a Letter of Credit. The Check represents the amount of uncompleted street work to be done in the Mayflower 6th Addition. Therefore, Mr. Leszczynski made a motion that the Check as submitted be accepted. Mrs. DeClercq seconded the motion which carried. APPROVE PROPOSAL FOR ELEVATOR MAINTENANCE Mr. Leszczynski advised that submitted to the Board was a Proposal for Elevator Maintenance as submitted by the Milar Elevator Service Company, P.O. Box 780, Toledo, Ohio to provide maintenance service for the two (2) Westinghouse geared passenger elevators at the South Bend Wayne Street Parking Garage at 121 East Wayne Street. Mr. Leszczynski noted that Milar is the current provider of this service and that the fee for this service is $1,100.00 net per month. Ms. Humphreys made a motion that the Proposal be approved and executed. Mr. Leszczynski seconded the motion which carried. APPROVE TRAFFIC CONTROL DEVICES Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and carried, the following two (2) traffic control devices were approved: 1. REMOVAL OF: Stop Sign LOCATION: Progress and Sheridan REMARKS: With new design of the street, stop sign is not needed.
