REGULAR MEETING OCTOBER 21, 1991 <br />Contract was approved and the appropriate Insurance, Performance <br />Bond and Labor and Materials Payment Bond as submitted were <br />filed. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - REMODELING OF NEW <br />EQUIPMENT SERVICES FACILITY - 1045 W. SAMPLE STREET <br />Mr. Leszczynski stated that L.L. Geans & Sons, Inc., 56535 <br />Magnetic Drive, Mishawaka, Indiana, has submitted the Project <br />Completion Affidavit for the above referred to project indicating <br />a final cost of $840,063.00. Upon a motion made by Mrs. <br />DeClercq, seconded by Mr. Leszczynski and carried the Project <br />Completion Affidavit was approved and the appropriate three-year <br />Maintenance Bond as submitted was filed. <br />APPROVE CHANGE ORDER NO. 6 - CONTRACT II - WASTEWATER TREATMENT <br />PLANT IMPROVEMENTS PROJECT <br />Mr. Leszczynski advised that Mr. Robert E. Toppel, Construction <br />Coordinator, Bureau of Wastewater, submitted to the Board Change <br />Order No. 6 (Final) for Contract II, Divisions A, B & C, in <br />regards to the Wastewater Treatment Plant Improvements as <br />executed by Calumet Construction. Change Order No. 6 indicates <br />an increase of $197,799.00 for a new Contract amount of <br />$7,938,239.50 and adds a time extension of two hundred twelve <br />(212) days to the project. Mrs. DeClercq made a motion that the <br />Change Order be approved and executed. Ms. Humphreys seconded <br />the motion which carried. <br />APPROVE REQUESTS TO ADVERTISE FOR THE RECEIPT OF BIDS: <br />ADDITIONAL METAL SECURITY SCREENING - ST. JOSEPH/WAYNE STREET <br />PARKING GARAGE <br />r FORD STREET AND LOMBARDY STREET SEWER LINERS <br />Mr Carl P. Littrell, Director, Division of Engineering, <br />requested that the Board advertise for the receipt of bids for <br />additional metal security screening at the St. Joseph/Wayne <br />Street Parking Garage on behalf of the Department of Economic <br />Development. Additionally, Mr. Littrell requested that the Board <br />advertise for the receipt of bids for the 1991 Ford Street and <br />Lombardy Street Sewer Liners on behalf of the Department of <br />Environmental Services. <br />Mr. Leszczynski made a motion that the request to advertise for <br />the receipt of bids for the additional metal security screening <br />at the parking garage be approved. Mrs. DeClercq seconded the <br />motion which carried. <br />Additionally, Mrs. DeClercq made a motion that the request to <br />advertise for the receipt of bids for the Ford Street and <br />Lombardy Street Sewer Liners Project be approved. Ms. Humphreys <br />seconded the motion which carried. <br />APPROVE REQUESTS: <br />- REQUEST OF HOPE RESCUE MISSION TO PLACE BICYCLE RACK ON PUBLIC <br />RIGHT-OF-WAY AT 532 SOUTH MICHIGAN STREET <br />- REQUEST OF THE WHITE RIBBON AGAINST PORNOGRAPHY CAMPAIGN TO <br />OCCUPY PUBLIC RIGHT-OF-WAY IN CONJUNCTION WITH RALLY - OCTOBER <br />27, 1991 <br />- REQUEST OF POTAWATOMI ZOOLOGICAL SOCIETY TO CHANGE WALL STREET <br />TO ONE-WAY IN CONJUNCTION WITH ZOO BOO ON OCTOBER 26-29 1991 <br />AND ZOOLTIDE DECEMBER 6-8, 13-15 AND 20-22, 1991 <br />- REQUEST OF PARK AVENUE NEIGHBORHOOD ASSOCIATION TO CLOSE <br />MANITOU, FROM FOREST AVENUE TO PARK AVENUE, FOR HALLOWEEN PARTY <br />OCTOBER 27, 1991 <br />Mr. Leszczynski advised that the appropriate City departments and <br />
