<br />Therefore, Mrs. Mueller made a motion that the request, as
<br />outlined above, be approved. Mr. Caldwell seconded the motion
<br />which carried.
<br />APPROVE CONSTRUCTION CONTRACT - PUMP NO. 3 DRIVE REPLACEMENT
<br />(WASTEWATER TREATMENT PLANT)
<br />In accordance with the bid awarded on December 7, 1992, to Shaum
<br />Electric Company, 1125 North Nappanee Street, Elkhart, Indiana,
<br />in the amount of $246,260.00 for the above referred to project, a
<br />Contract in said amount was submitted for Board approval. Upon a
<br />motion made by Mrs. Mueller, seconded by Mr. Caldwell and
<br />carried, the Contract was approved and the appropriate
<br />Certificate of Insurance, Performance Bond and Labor and
<br />Materials Payment Bond as submitted were filed.
<br />APPROVE ADDENDUM TO LIMITED ADMINISTRATORS AGREEMENT (LINCOLN
<br />NATIONAL)
<br />It was noted that this is an amendment to our current contract
<br />with Lincoln National for flex account administration. Employers
<br />Health Insurance (EHIC) is the successor to Lincoln National on
<br />this contract. This extends the administration period for ninety
<br />(90) days for a fee of $8,157.60. Upon a motion made by Ms.
<br />Mueller, seconded by Mr. Leszczynski and carried, the addendum
<br />was approved.
<br />AWARD BID - FORD STREET SEWER LINERS 1992
<br />In a Memorandum to the Board, Mr. Carl P. Littrell, Director,
<br />Division of Engineering, advised that on December 14, 1992, the
<br />Board received and opened one (1) bid for the above referenced
<br />project. Mr. Littrell stated that he has reviewed this one (1)
<br />bid and found that the amount of the bid agrees with the amount
<br />read at the Board meeting. Therefore, Mr. Littrell recommended
<br />that the Board award the bid of Insituform Midwest, Inc., 111th &
<br />Archer, P.O. Box 458, Lemont, Illinois, in the amount of
<br />$79,292.50. Mr. Littrell noted that the unit prices offered on
<br />this Contract are in line with prices offered for the same kind
<br />of work one (1) year ago. Insituform Midwest performed that work
<br />successfully and efficiently. Therefore, Mr. Caldwell made a
<br />motion that the recommendation be accepted and the bid be
<br />awarded. Mrs. Mueller seconded the motion which carried.
<br />AWARD QUOTATION - WATERPROOFING OF COVELESKI REGIONAL STADIUM
<br />Mr. Bob Allen, Manager, Bureau of Public Construction, advised
<br />that on December 14, 1992, the Board received Quotations for the
<br />waterproofing of the split face block at the Coveleski Regional
<br />Stadium. Mr. Allen noted that he has reviewed the Quotations and
<br />found that the amounts agree with the amounts read at the
<br />meeting.
<br />Mr. Allen noted that Advanced Masonry Construction Company, Inc.,
<br />53266 Bonvale Drive, South Bend, Indiana, submitted the low
<br />Quotation in the amount of $13,867.00. However, they have asked
<br />to withdraw their Quotation as it contained a quantity
<br />calculation error.
<br />Therefore, Mr. Allen recommended that the Board award the next
<br />low Quotation submitted by Casteel Construction Corporation,
<br />23186 West Ireland Road, South Bend, Indiana, in the amount of
<br />$23,892.00.
<br />Mrs. Mueller made a motion that the recommendation be accepted
<br />and the Quotation be awarded. Mr. Caldwell seconded the motion
<br />which carried.
