REGULAR MEETING NOVEMBER 21. 1994 The regular meeting of the Board of Public Works was convened at 9:35 a.m., on Monday, November 21, 1994, by Board President John E. Leszczynski, with Mr. Leszczynski, Mr. James R. Caldwell and Mrs. Jenny Pitts Manier present. Also present was Board Attorney Anne Bruneel. AGENDA ITEMS ADDED Upon a motion made by Mr. Caldwell, seconded by Mr. Leszczynski and carried, agenda items to award a quotation and to request approval of recommendations were added. APPROVE MINUTES OF PREVIOUS MEETING Upon a motion made by Mr. Caldwell, seconded by Mr. Leszczynski and carried, the minutes of the regular meeting of the Board held on November 14, 1994, were approved. OPENING OF BIDS - ROOF REPAIRS AT CENTURY CENTER - PROJECT NO. 94-85 This was the date set for receiving and opening of sealed bids for the above referred to project. The Clerk tendered proofs of publication of Notice in the South Bend Tribune and the Tri-County News which were found to be sufficient. The following bids were opened and publicly read: MIDLAND ENGINEERING COMPANY 52369 U.S. 33 North South Bend, Indiana 46624 Bid was signed by Mr. Charles W. Frazier, President/CEO Non -Collusion Affidavit was in order Non -Discrimination Commitment form was completed Five per cent (5%) Bid Bond was submitted BID: $489,680.00 Alternate R-1 Add $ 21,000.00 Alternate R-2 Add $106,600.00 Alternate R-3 Add $ 86,915.00 Clarifications - To remove and replace existing counterflashing per detail 6/A-7, add $8.50/lf. Asbestos abatement removal and disposal is based on the plans and specifications and current EPA and OSHA guidelines. Base bid work will be completed by June 1, 1995. CITY ROOFING, INC. 2300 Sterling Avenue Elkhart, Indiana 46516 Bid was signed by Mr. James N. Purvis, President Non -Collusion Affidavit was in order Non -Discrimination Commitment form was completed Five per cent (5%) Bid Bond was submitted BID: $537,900.00 Alternate R-1 Add $ 14,500.00 Alternate R-2 Add $119,400.00 Alternate R-3 Add $ 90,000.00 Upon a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the above bids were referred to Mathews-Purucker-Anella, 218 S. Frances Street, South Bend, Indiana, for review and recommendation.
