McNease 439 South Michigan Street South Bend, Indiana 46601 Lot located at: 418 South Main Street APPROVE APPLICATION FOR PERMIT - SPECIALIZED COLLECTION SERVICES Mr. Leszczynski advised that pursuant to the terms of the trash and rubbish removal ordinance, the Board is in receipt of a Permit application from a private trash hauler who desires to continue his business within the City. The following application has been submitted: 1. Mr. Roosevelt Lacy LACY'S RUBBISH SERVICE 2143 North Elmer Street South Bend, Indiana 46628 It is noted that attached to the application was a list of individuals to which this vendor will provide services. Mr. Leszczynski made a motion that this Permit application be approved. Mr. Caldwell seconded the motion which carried. FILING OF CERTIFICATES OF INSURANCE Upon a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, Certificates of Insurance for the following were accepted for filing: 1. Warner & Sons, Inc. P.O. Box 87 Elkhart, Indiana 46515 2. Northcutt Tree Service Inc. 51831 CR 11 Elkhart, Indiana 46514 FILING OF HUMANE SOCIETY MONTHLY REPORT - DECEMBER 1993 Mr. Leszczynski indicated that the Board is in receipt of the monthly report as submitted by the Humane Society of St. Joseph County, Inc., for the month of December, 1993. The report indicates the type and number of animals handled for the City of South Bend. Mr. Leszczynski made a motion that the report be accepted for filing as submitted. Mrs. Manier seconded the motion which carried.
