<br />noted that the Agreement is in the amount of $2,900.00. Therefore, upon a motion made by Mr.
<br />Caldwell, seconded by Mr. Leszczynski and carried, the Agreement was approved and executed.
<br />APPROVAL OF AGREEMENT - AUDIT SERVICES FOR GRANTS - CROWE CHIZEK
<br />Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend
<br />and Crowe Chizek, 330 East Jefferson Boulevard, South Bend, Indiana, for audit services for the
<br />City of South Bend Department of Community and Economic Development. The audit projects are
<br />for William Richardson Court and Build Indiana Project, and Crowe Chizek proposes to audit the
<br />financial statements of the each of the grants for the specific grant period. Mr. Leszczynski noted
<br />that the Agreement is in the amount of $500.00 to $750.00 per grant. Therefore, upon a motion
<br />made by Mr. Caldwell, seconded by Mr. Leszczynski and carried, the Agreement was approved and
<br />executed.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />STUDEBAKER NATIONAL MUSEUM - TEMPORARY FENCING - PROJECT NO 95-46
<br />Mr. Leszczynski advised that Mr. Carl P. Littrell, P.E., Director, Division of Engineering has
<br />submitted Change Order No. 1 (Final), on behalf of Schuell Fence Company, 22377 Adams Road,
<br />South Bend, Indiana, indicating that the contract amount be increased by $90.00 for a new contract
<br />sum including this Change Order in the amount of $7,390.00. Additionally submitted was the
<br />Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Caldwell,
<br />seconded by Mr. Leszczynski and carried, Change Order No. 1 (Final) and the Project Completion
<br />Affidavit were approved subject to the filing of the appropriate three-year Maintenance Bond.

<br />Mr. Leszczynski advised that Mr. Bob Allen, Manger, Public Construction, has submitted Change
<br />Orders No. 4, 5 and 6, on behalf of Advanced Pollution Technologists, Limited, 3505 North Home
<br />Street, Mishawaka, Indiana, and recommended approval as follows:

<br />Upon a motion made by Mr. Caldwell, seconded by Mr. Leszczynski and carried, the Change Orders

<br />APPROVE CHANGE ORDER - STUDEBAKER NATIONAL MUSEUM - ELECTRICAL
<br />REPAIRS - PROJECT NO.95-48
<br />Mr. Leszczynski advised that Mr. Carl P. Littrell, P.E., Director, Division of Engineering, has
<br />submitted Change Order No. 1, on behalf of McDaniel Fire Systems, 1055 West Joliet Road,
<br />Valparaiso, Indiana, indicating that the Contract amount be increased $0.00 for a new Contract sum.
<br />by including this Change Order in the amount of $14,966.00. Upon a motion made by Mr.
<br />Leszczynski, seconded by Mr. Caldwell and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER - COLLEGE FOOTBALL HALL OF FAME - THEME
<br />SCULPTURE - MUSEUM SERVICES INC.
<br />Mr. Leszczynski advised that Mr. David Fox, Hilferty & Associates, 14240 Route 550, Athens,
<br />Ohio, has submitted Change Order No. CO #2, on behalf of Museum Services, Inc.,1220 SE Veitch
<br />Street, Gainesville, Florida indicating that the Contract amount be increased $851.70 for a new
