REGULAR MEETING <br />JUNE 12. 1995 <br />3. That this Resolution shall be in full force and effect upon its adoption by the Board <br />of Public Works of the City of South Bend. <br />Adopted at a meeting of the Board of Public Works of the City of South Bend held on June <br />12, 1995, at 1308 County -City Building, 227 West Jefferson Boulevard, South Bend, Indiana 46601. <br />CITY OF SOUTH BEND <br />BOARD OF PUBLIC WORKS <br />s/John E. Leszczynski, President <br />s/Jenny Pitts Manier, Member <br />ATTEST: <br />s/Angela K. Jacob, Clerk <br />APPROVE CONTRACTOR AND EXCAVATION BONDS <br />Mr. Larry D. Spradlin, Sr., Engineer, Division of Engineering, recommended that the following <br />Contractors and Excavation Bonds be approved as follows: <br />BOND OF CONTRACTOR <br />Terry Horein Builders Approved Effective June 12, 1995 <br />EXCAVATION BONDS <br />L.E. Myers Company Approved Effective June 12, 1995 <br />Mr. Leszczynski made a motion that the recommendation be accepted and that the Bonds be <br />approved and/or released as outlined above. Mrs. Manier seconded the motion which carried. <br />APPROVE TRAFFIC CONTROL DEVICE <br />Upon a motion made by Mrs. Manier, seconded by Mr. Leszczynski and carried, the following traffic <br />control device was approved: <br />REMOVAL OF: Remove 1 hour parking 6 a.m. - 6 p.m. <br />REVISED TO: Loading Zone <br />LOCATION: 1814 South Main <br />REMARKS: Michiana Construction Supply request <br />FILING OF CERTIFICATE OF INSURANCE <br />Upon a motion made by Mrs. Manier, seconded by Mr. Leszczynski and carried, the following <br />Certificate of Insurance was accepted for filing: <br />Saint Joseph's Medical Center <br />801 East LaSalle Street <br />South Bend, Indiana 46634 (Sportsmed 10-K) <br />APPROVE CLAIMS <br />Mr. Kevin Horton, City Controller, submitted claims in the amount of $1,053,965.31 and <br />recommended approval. Additionally, Ms. Ann Kolata, Department of Community and Economic <br />Development, submitted claims for the College Football Hall of Fame and recommended approval. <br />Mr. Leszczynski made a motion that the claims be approved and the reports as submitted be filed. <br />Mrs. Manier seconded the motion which carried. <br />
