APPROVE CONSTRUCTION CONTRACT - EDISON ROAD SANITARY SEWER EXTENSION - PROJECT NO. 5-102 Mr. Leszczynski advised that in accordance with the bid awarded on January 15, 1996, to Herrman & Goetz, Inc., d/b/a Underground Services, 225 South Lafayette, South Bend, Indiana, in the amount of $81,059.63 for the above referred to project, a Contract in said amount was being submitted for Board approval. Upon a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the Contract was approved and the appropriate Certificate of Insurance, Performance Bond and Labor and Materials Payment Bond as submitted were filed. APPROVE PROJECT COMPLETION AFFIDAVIT - WASTEWATER TREATMENT GRIT CHANNEL REBUILD - PROJECT NO. 95-75A Mr. Leszczynski advised that Mr. Roberto Flores, Environmental Services, has submitted the Project Completion Affidavit on behalf of Industrial Installations, 55587 Currant Road, Mishawaka, Indiana, for the above referred to project, indicating a final cost of $35,008.00. Upon a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the Project Completion Affidavit was approved. APPROVAL OF AGREEMENT - MENTAL HEALTH PROVIDER - MADISON CENTER Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend and Madison Center, 403 East Madison, South Bend, Indiana, for providing mental health and substance abuse services for the City of South Bend employees. Therefore, upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the Agreement was approved and executed. APPROVAL OF GRANT OF EASEMENT Mr. Leszczynski stated that the Board is in receipt of a Grant of Easement between the City of South Bend and Jim and Michelle Niespodzainy, on their property, described as:
