<br />Upon a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the above bids
<br />were referred to the Department of Code Enforcement for review and recommendation.
<br />AWARD BID - REMOVAL OF UNDERGROUND STORAGE TANKS AND DISPENSING
<br />EQUIPMENT - PROJECT. NO. 95-73
<br />Mr. Carl P. Littrell, Director, Division of Engineering, advised the Board that on December 4, 1995,
<br />bids were received and opened for the above referred to project. After reviewing those bids, Mr.
<br />Littrell recommends that the Board award the contract to Knapp Environmental Construction
<br />Services, Inc., 12875 McKinley Highway, Mishawaka, Indiana, in the amount of $39,275.00.
<br />Therefore, Mrs. Manier made a motion that the recommendation be accepted and the bid be awarded
<br />as outlined above. Mr. Caldwell seconded the motion which carried.
<br />APPROVAL OF AGREEMENT - CONSULTING AGREEMENT - GIBSON RISK
<br />MANAGEMENT SERVICES; INC.
<br />Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend
<br />and Gibson Risk Management Services, Inc., 300 St. Louis Boulevard, South Bend, Indiana, for
<br />insurance consulting services. Mr. Leszczynski noted that the Agreement is in the amount of
<br />$24,000.00, and payment will be billed and payable on January 31, 1996, in the amount of
<br />$2,000.00, and each month thereafter through December 31, 1996. Therefore, upon a motion made
<br />by Mrs. Manier, seconded by Mr. Caldwell and carried, the Agreement was approved and executed.
<br />APPROVAL OF AGREEMENT - CONSULTING AGREEMENT - GIBSON RISK
<br />MANAGEMENT SERVICES, INC.
<br />Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend
<br />and Gibson Risk Management Services, Inc., 300 St. Louis Boulevard, South Bend, Indiana, for
<br />insurance consulting services. Mr. Leszczynski noted that the Agreement is in the amount of
<br />$18,000.00, and payment will be billed and payable on January 31, 1996, in the amount of
<br />$1,500.00, and each month thereafter through December 31, 1996. Therefore, upon a motion made
<br />by Mrs. Manier, seconded by Mr. Caldwell and carried, the Agreement was approved and executed.
<br />APPROVAL OF AGREEMENT - ESCROW AGREEMENT - INSTALLATION OF EXHAUST
<br />SYSTEMS AT SOUTH BEND FIRE STATIONS - PROJECT NO. 95-84
<br />Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend
<br />and O.J. Shoemaker, 1008 Lincolnway East, South Bend, Indiana, and First National Bank of
<br />Monterrey, Post Office Box 8, Monterrey, Indiana, for a Construction Contract for the above
<br />referred to project, which is in excess of $100,000.00. Therefore, upon a motion made by Mr.
<br />Caldwell, seconded by Mrs. Manier and carried, the Agreement was approved and executed.
<br />APPROVAL OF WARRANTY - INSTALLATION OF UNDERGROUND FUEL STORAGE
<br />TANKS AND DISPENSING EQUIPMENT - PROJECT NO. 95-73
<br />Mr. Leszczynski stated that the Board is in receipt of a Warranty for the above referred to project,
<br />as submitted by Advance Pollution Technologists, Ltd., 3505 North Home Street, Mishawaka,
<br />Indiana. This warranty certifies the installation of the underground storage tanks. Upon a motion
<br />made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the Warranty was approved.
<br />APPROVE CONTRACT FOR EMERGENCY MEDICAL SERVICES - UNIVERSITY OF
<br />NOTRE DAME/SOUTH BEND FIRE DEPARTMENT
<br />Mr. Leszczynski advised that the Board is in receipt of a Contract for Emergency Medical Services
<br />between the City of South Bend Fire Department, and the University of Notre Dame by and through
<br />the Board of Public Works. Mr. Leszczynski stated that the South Bend Fire Department will
<br />provide to the University the following emergency medical equipment and personnel for the events
<br />specified by the University:
