REGULAR MEETING NOVEMBER 1, 1999 <br />APPROVE CHANGE ORDER NO. 1 FOR SOUTH MICHIGAN STREETSCAPE <br />IWROVEMENTS - PROJECT NO. 99-59 <br />Mr. Caldwell advised that Mr. Carl Littrell, P.E., Division of Engineering submitted Change Order <br />No. 1 on behalf of Rieth-Riley Construction Co., Inc., indicating that an increase in the contract <br />amount by $581.69, with anew contract sum of $233,949.69. Upon a motion made by Ms. Roemer, <br />seconded by Mr. Powell and carried, the Change Order was approved. <br />APPROVE CHANGE ORDER FOR ADDITIONAL SERVICES AGREEMENT NO. 1, SOUTH <br />BEND PUBLIC WORKS SERVICE CENTER PRELIMINARY SITE SELECTION ANALYSIS <br />Mr. Caldwell advised that Mr. Carl Littrell, P.E., Division of Engineering submitted Change Order <br />No. One on behalf of Herceg & Associates, indicating that an increase in the contract amount from <br />$59,100.00 to the new contract sum of $61,500.00. Upon a motion made by Ms. Roemer, seconded <br />by Mr. Powell and carried, the Change Order was approved. <br />APPROVE CHANGE ORDER NO. 003 FOR MORRIS PERFORMING ARTS CENTER <br />MASONRY & MASONRY RESTORATION - PROJECT NO. 98-25-5.4 <br />Mr. Caldwell advised that Mr. Carl Littrell, P.E., Division of Engineering submitted Change Order <br />No. 003 on behalf of Ramenda Masonry Co., Inc., indicating an increase in the contract amount by <br />$7,200.00 with a new contract sum of $562,615.00. Upon a motion made by Mr. Powell, seconded <br />by Mr. Caldwell and carried, the Change Order was approved. <br />APPROVE CONSENT TO ANNEXATION AND WAIVER OF RIGHTS TO REMONSTRATE <br />Mr. Caldwell stated that the Board is in receipt of a Consent to Annexation and Waiver of Right to <br />Remonstrate as submitted by VGB LLC. The Consent indicates that in consideration for permission <br />to tap into the public sanitary sewer system of the City, to provide sanitary sewer service to 51900 <br />Mayflower Road, South Bend, Indiana, (Key # 04-1009-0158.01), the owners and successors waive <br />and release any and all right to remonstrate against or oppose any pending or future annexation of <br />the property by the City of South Bend. Therefore, Ms. Roemer made a motion that the Consent be <br />approved. Mr. Powell seconded the motion which carried. <br />APPROVE CONSENT TO ANNEXATION AND WAIVER OF RIGHTS TO REMONSTRATE <br />Mr. Caldwell stated that the Board is in receipt of a Consent to Annexation and Waiver of Right to <br />Remonstrate as submitted by Stephen R Troeger. The Consent indicates that in consideration for <br />permission to tap into the public sanitary sewer system of the City, to provide sanitary sewer service <br />to 52826 Brandel Avenue, South Bend, Indiana, (Key # 02-2162-7859), the owners and their <br />successors waive and release any and all right to remonstrate against or oppose any pending or future <br />annexation of the property by the City of South Bend. Therefore, Ms. Roemer made a motion that <br />the Consent be approved. Mr. Powell seconded the motion which carried. <br />APPROVE CONSENT TO ANNEXATION AND WAIVER OF RIGHTS TO REMONSTRATE <br />Mr. Caldwell stated that the Board is in receipt of a Consent to Annexation and Waiver of Right to <br />Remonstrate as submitted by William A. and David L. Geyer. The Consent indicates that in <br />consideration for permission to tap into the public sanitary sewer system of the City, to provide <br />sanitary sewer service to property on Northside Boulevard, (Key # 01-1017-0337), the owners and <br />their successors waive and release any and all right to remonstrate against or oppose any pending <br />or future annexation of the property by the City of South Bend. Therefore, Mr. Caldwell made a <br />motion that the Consent be approved. Mr. Powell seconded the motion which carried. <br />FILING OF CERTIFICATE OF INSURANCE <br />Upon a motion made by Mr. Caldwell, seconded by Ms. Roemer and carried, the following <br />Certificates of Insurance were accepted for filing: <br />James Jankowski with Carnegie Insurance Service Corporation for coverage on 1-91 Ford <br />Aerostar. <br />Northern Indiana Construction with Waterfield Insurance and additional insured being the <br />City of South Bend. <br />
