<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — 2005
<br />STREET MATERIALS — PROJECT NO. 105-042 (LRSA)
<br />In a memorandum to the Board, Mr. Jason Durr, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referred to project. Therefore, upon a
<br />motion made by Mr. Gilot, seconded by Mr. Inks, the above request was approved.
<br />APPROVAL OF REQUEST FOR SOLICITATION_ OF PROPOSALS — COLLECTION
<br />OF PAST DUE AMBULANCE ACCOUNTS FOR 2005 — SOUTH BEND FIRE
<br />DEPARTMENT
<br />In a memorandum to the Board, Assistant Fire Chief Rick Switalski, requested
<br />permission to advertise for proposals for the above referred to project. Therefore, upon a
<br />motion made by Mr. Gilot, seconded by Mr. Littrell, the above request was approved.
<br />APPROVE CHANGE ORDER — RIVER PARK PARTNERSHIP CENTER PUBLIC
<br />WORKS SERVICE AREAS — KASER SPRAKER CONSTRUCTION — PROJECT NO
<br />105-030 (GENERAL FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on
<br />behalf of Kaser Spraker Construction, 25487 West State Road 2, South Bend, Indiana,
<br />indicating that the Contract amount be increased $2,460.00 for a new Contract sum
<br />including this Change Order in the amount of $18,796.00. Upon a motion made by Mr.
<br />Gilot, seconded by Mr. Littrell, and carried, the Change Order was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — COUNTY CITY BUILDING
<br />14TH FLOOR RENOVATION — KASER SPRAKER CONSTRUCTION — PROJECT
<br />NO. 104-076 (GENERAL FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the
<br />Project Completion Affidavit on behalf of Kaser Spraker Construction, Post Office Box
<br />3605, South Bend, Indiana, for the above referred to project, indicating a final cost of
<br />$20,950.00. Upon a motion made by Mr. Gilot, seconded by Mr. Inks, and carried, the
<br />Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — COUNTY CITY BUILDING
<br />14TH FLOOR RENOVATION — CARPETING — MIDWEST TILE & INTERIORS —
<br />PROJECT NO. 104-076 (GENERAL FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the
<br />Project Completion Affidavit on behalf of Midwest Tile & Interiors, Inc., 1805 North
<br />Cedar, Mishawaka, Indiana, for the above referred to project, indicating a final cost of
<br />$26,205.00. Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the
<br />Project Completion Affidavit was approved.
